Sweden meets all those streaming requirements you listed (not planes yet but yes on subways, trains, cars, and rural areas). Here in Sweden anyone who wants it can get 100Mb (and some 1Gb) unlimited and priced reasonably wired broadband internet, or Turbo 3G wireless. Norway, Denmark and Finland are not far behind this bandwidth bliss we enjoy in Sweden. PS!
